We’ll guide you through each step and help you make confident, informed choices about your housing options.
The form is easy to complete electronically. We can email or post a copy to you if you prefer.
Before you begin:
Save the file to your device and select 'Enable Editing' to complete it.
Type directly into the boxes and add your electronic signature using the “Freeform: Scribble” tool in Word.
When finished, save your completed form and attach it to an email with your supporting documents.

For Extra Care Housing a recent Adult Social Care needs assessment provided by Lincolnshire County Council.

This depends on availability and the type of housing you’ve applied for.
We don’t give numbered positions on the waiting list because these change depending on priority banding and for Extra Care Housing the care/support balance within each scheme.
We’ll contact you when a suitable vacancy arises that matches your preferences.
Yes. A family member, advocate, or carer can help you complete your application — we’ll just need your written permission to share information with them.
However, you must have the capacity to sign the tenancy agreement yourself and understand its terms.
If we have any concerns about this, we’ll discuss them sensitively and make sure the right advice and support are available.
Once your application is accepted, we’ll review it carefully and match you with available homes based on suitability and priority.
We acknowledge applications within five working days and assess them within 21. We look at your housing needs, finances, and preferences to find the best fit for you.
Properties are offered according to availability within the schemes or developments you’ve registered for and your priority banding at the time.
If a suitable home becomes available, we’ll arrange a meeting to confirm eligibility and explain rent, charges, and tenancy terms.
Our Pre-Tenancy and Affordability Services help you prepare for move-in — from setting up a Direct Debit to understanding your tenancy or lease in full.
